Serwis – serwis internetowy SOSlink oraz jego funkcjonalności dostępne online, w tym sklep internetowy i formularze powiązane z usługą SOSlink.

Formularz ICE – formularz udostępniany użytkownikowi po zakupie albo w ramach modelu eventowego, służący do podania danych, które mają zostać wykorzystane do przygotowania Naklejki SOS i kodu QR.

Naklejka SOS – spersonalizowany produkt lub rezultat usługi SOSlink zawierający kod QR oraz dane podane przez użytkownika w zakresie przez niego wybranym.

Organizator – podmiot organizujący wydarzenie, w ramach którego użytkownik może skorzystać z usługi SOSlink w modelu eventowym.

Wydarzenie – wydarzenie sportowe albo inne przedsięwzięcie organizowane przez Organizatora, w związku z którym użytkownik otrzymuje dostęp do usługi SOSlink.

Model eventowy – sposób korzystania z usługi SOSlink, w którym użytkownik otrzymuje dostęp do formularza ICE w związku z udziałem w Wydarzeniu organizowanym przez Organizatora.

Model B2C – sposób korzystania z usługi SOSlink, w którym użytkownik samodzielnie zamawia produkt lub usługę bezpośrednio przez Serwis.

Dane dotyczące zdrowia – dane dotyczące zdrowia w rozumieniu przepisów o ochronie danych osobowych, podawane dobrowolnie przez użytkownika w formularzu ICE.

Użytkownik – osoba korzystająca z Serwisu, składająca zamówienie lub wypełniająca formularz ICE.

2. Informacje ogólne

Niniejsza Polityka prywatności określa zasady przetwarzania danych osobowych w związku z korzystaniem z serwisu internetowego SOSlink, realizacją zamówień składanych przez stronę internetową oraz korzystaniem z formularza ICE służącego do przygotowania Naklejki SOS i kodu QR. Polityka rozróżnia dane przetwarzane na etapie sklepu i płatności od danych podawanych w formularzu ICE, w tym danych dotyczących zdrowia.

Polityka prywatności dotyczy danych osobowych przetwarzanych w związku z:

WAŻNE INFORMACJE: Usługa SOSlink działa co do zasady dwuetapowo. W pierwszym etapie użytkownik składa zamówienie i dokonuje płatności. W drugim etapie po zakupie albo w ramach modelu eventowego użytkownik otrzymuje dostęp do formularza ICE, w którym może dobrowolnie podać dane, które mają znaleźć się na Naklejce SOS, w tym dane dotyczące zdrowia. Dane dotyczące zdrowia są przetwarzane w odrębnym, węższym zakresie niż dane związane z obsługą sklepu, płatności lub plików cookies.

4. Model usługi

Usługa SOSlink może działać w dwóch modelach:

model eventowy – gdy użytkownik otrzymuje link do formularza w związku z wydarzeniem organizowanym przez Organizatora,
model B2C – gdy użytkownik samodzielnie zamawia usługę lub produkt przez stronę internetową.

W modelu eventowym Organizator pozostaje odrębnym administratorem danych, które przetwarza na potrzeby organizacji wydarzenia we własnym zakresie. Administrator SOSlink przetwarza dane osobowe samodzielnie na potrzeby usługi SOSlink.

W modelu bezpośrednim (B2C) proces może przebiegać dwuetapowo: najpierw użytkownik składa zamówienie i dokonuje płatności, a następnie otrzymuje link do formularza ICE, w którym może uzupełnić dane, które mają znaleźć się na Naklejce SOS. Dane podawane na etapie zamówienia i dane podawane w formularzu ICE mogą podlegać odmiennym zasadom przetwarzania, w szczególności w odniesieniu do danych dotyczących zdrowia.

Zgodnie z przyjętym modelem Organizator nie otrzymuje dostępu do danych źródłowych wpisanych do formularza SOSlink, a jedynie gotowy rezultat usługi, w zakresie niezbędnym do wydruku, dystrybucji lub wydania produktu.

Dane dotyczące zdrowia nie są elementem procesu płatności ani standardowej analityki sklepu.

5. Jakie dane przetwarzamy

W zależności od etapu korzystania z usługi możemy przetwarzać następujące dane:

dane identyfikacyjne i kontaktowe, takie jak imię, nazwisko, adres e-mail, numer telefonu, adres dostawy,
dane związane z realizacją zamówienia, płatnością, dostawą, reklamacją i rozliczeniem,
dane techniczne związane z korzystaniem z Serwisu, takie jak adres IP, identyfikatory urządzenia, dane o sesji lub plikach cookies – w zakresie wynikającym z ustawień Serwisu i przeglądarki.

Dane przetwarzane na etapie formularza ICE:

dane niezbędne do przygotowania Naklejki SOS i kodu QR,

dane dotyczące zdrowia podane dobrowolnie przez użytkownika w formularzu ICE.

Dane techniczne, analityczne i cookies dotyczą korzystania z Serwisu i etapu sklepu. Nie wykorzystujemy ich do opisu, profilowania ani analityki danych dotyczących zdrowia podanych w formularzu ICE.

6. Skąd mamy dane

Co do zasady dane osobowe otrzymujemy bezpośrednio od Ciebie.

Na etapie sklepu i zamówienia dane otrzymujemy, gdy:

korzystasz z Serwisu,
składasz zamówienie,
dokonujesz płatności,
kontaktujesz się z nami,
składasz reklamację.

Na etapie formularza ICE dane otrzymujemy, gdy:

wypełniasz formularz ICE,
podajesz dane do Naklejki SOS i kodu QR,
dobrowolnie podajesz dane dotyczące zdrowia.

Dane dotyczące zdrowia nie są pobierane na etapie samego zakupu, lecz wyłącznie na etapie formularza ICE.

Jeżeli usługa dotyczy dziecka albo innej osoby reprezentowanej, dane mogą zostać przekazane przez rodzica, opiekuna prawnego albo inną osobę uprawnioną do działania w jej imieniu.

W modelu eventowym możemy ponadto przetwarzać informacje techniczne lub organizacyjne związane z przypisaniem formularza do określonego wydarzenia albo sposobu wydania gotowego produktu, jeżeli taki model został wdrożony w praktyce.

7. Czy podanie danych jest obowiązkowe

Podanie danych niezbędnych do złożenia zamówienia, płatności, dostawy, kontaktu lub reklamacji jest dobrowolne, ale konieczne do realizacji zamówienia albo wykonania umowy.

Podanie danych w formularzu ICE jest dobrowolne w zakresie, w jakim użytkownik sam decyduje, jakie informacje chce umieścić na Naklejce SOS.

Podanie danych dotyczących zdrowia jest całkowicie dobrowolne. Ich niepodanie nie uniemożliwia co do zasady zakupu produktu lub skorzystania z usługi, ale może spowodować, że Naklejka SOS zostanie przygotowana w wariancie podstawowym albo z ograniczoną funkcjonalnością.

8. Cele i podstawy prawne przetwarzania

Twoje dane osobowe przetwarzamy w następujących celach:

Informacje dotyczące plików cookies, danych technicznych i podobnych technologii odnoszą się do korzystania z Serwisu i etapu sklepu. Nie wykorzystujemy tych technologii do przetwarzania danych dotyczących zdrowia podanych w formularzu ICE.

9. Komu możemy ujawniać dane

Dane sklepu i zamówienia mogą być ujawniane podmiotom, które wspierają Administratora w świadczeniu usług lub realizacji zamówień, w szczególności:

dostawcom hostingu, infrastruktury IT, chmury lub wsparcia technicznego,
dostawcom narzędzi wykorzystywanych do obsługi Serwisu i komunikacji,
operatorom płatności – w przypadku zamówień płatnych,

Dane z formularza ICE, w tym dane dotyczące zdrowia, mogą być ujawniane wyłącznie podmiotom niezbędnym do technicznej realizacji usługi SOSlink, w szczególności:

dostawcom infrastruktury IT i utrzymania technicznego formularza ICE,
podmiotom wspierającym techniczne przygotowanie Naklejki SOS lub kodu QR wyłącznie w zakresie niezbędnym do wykonania usługi,
podmiotom uprawnionym na podstawie przepisów prawa.

Dane dotyczące zdrowia nie są przekazywane operatorom płatności ani podmiotom realizującym analitykę lub działania marketingowe.

W modelu eventowym Organizator otrzymuje wyłącznie gotowy rezultat usługi w zakresie niezbędnym do wydania lub dystrybucji produktu. Zgodnie z przyjętym procesem po stronie SOSlink nie dochodzi do udostępnienia Organizatorowi danych źródłowych wpisanych do formularza.

10. Czy przekazujemy dane poza EOG

Co do zasady dążymy do tego, aby dane były przetwarzane na terenie Europejskiego Obszaru Gospodarczego.

Jeżeli w związku z korzystaniem z określonych dostawców lub narzędzi dojdzie do przekazania danych poza EOG, nastąpi to wyłącznie zgodnie z przepisami prawa, z zastosowaniem odpowiednich mechanizmów legalizujących taki transfer, w szczególności standardowych klauzul umownych albo innego właściwego mechanizmu przewidzianego przez RODO.

11. Jak długo przechowujemy dane

Okres przechowywania danych zależy od celu przetwarzania.

a) model eventowy

Dane podane w formularzu przetwarzamy przez okres niezbędny do wykonania usługi. Zgodnie z przyjętym modelem procesowym dane te są co do zasady usuwane najpóźniej w 7dni po wydarzeniu, z zastrzeżeniem danych, które muszą być przechowywane dłużej na podstawie przepisów prawa albo na potrzeby ustalenia, dochodzenia lub obrony roszczeń.

b) model B2C

Dane związane z zamówieniem przechowujemy przez okres niezbędny do realizacji zamówienia, rozliczeń, obsługi reklamacji, wykonania obowiązków prawnych oraz przez okres niezbędny do obrony przed roszczeniami lub dochodzenia roszczeń.

c) dane dotyczące zdrowia

Dane dotyczące zdrowia przechowujemy nie dłużej niż jest to niezbędne do wykonania usługi, chyba że dłuższe przechowywanie wynika z przepisów prawa albo jest niezbędne do ochrony roszczeń. Wycofanie zgody nie wpływa na zgodność z prawem przetwarzania, którego dokonano przed jej wycofaniem.

d) dane związane z kontaktem

Dane podane w wiadomościach lub reklamacjach przechowujemy przez okres niezbędny do obsługi sprawy, a następnie przez okres właściwy dla obrony przed roszczeniami lub dochodzenia roszczeń.

Jeżeli podstawą przetwarzania danych dotyczących zdrowia jest Twoja zgoda, możesz ją wycofać w dowolnym momencie, kontaktując się z Administratorem.

Wycofanie zgody nie wpływa na zgodność z prawem przetwarzania dokonanego przed jej wycofaniem ani na przetwarzanie innych danych niezbędnych do realizacji zamówienia, rozliczeń, obsługi reklamacji lub wykonania obowiązków prawnych. Może jednak spowodować brak możliwości dalszego wykonywania usługi w zakresie, który wymaga przetwarzania danych dotyczących zdrowia.

14. Zautomatyzowane podejmowanie decyzji i profilowanie

Co do zasady Twoje dane osobowe nie są wykorzystywane do zautomatyzowanego podejmowania decyzji wywołującego wobec Ciebie skutki prawne ani do profilowania w rozumieniu RODO.

Jeżeli w przyszłości wdrożone zostaną rozwiązania powodujące takie skutki, Polityka prywatności zostanie odpowiednio uzupełniona.

15. Dane dzieci i osób reprezentowanych

Jeżeli usługa dotyczy dziecka albo innej osoby reprezentowanej, dane powinny być przekazywane wyłącznie przez rodzica, opiekuna prawnego albo inną osobę uprawnioną do działania w jej imieniu.

Administrator może przetwarzać zarówno dane osoby, której dotyczy usługa, jak i dane osoby składającej zamówienie, formularz lub oświadczenie.

16. Pliki cookies i podobne technologie

Serwis może wykorzystywać pliki cookies lub podobne technologie przede wszystkim na etapie korzystania ze strony internetowej, sklepu i funkcji związanych z obsługą zamówienia.

Pliki te mogą służyć w szczególności do:

zapewnienia prawidłowego działania Serwisu,
utrzymania sesji użytkownika,
zwiększenia bezpieczeństwa,
zapamiętania ustawień użytkownika,

Administrator stosuje odpowiednie środki techniczne i organizacyjne mające na celu ochronę danych osobowych przed ich utratą, zniszczeniem, nieuprawnionym ujawnieniem albo nieuprawnionym dostępem.

W ramach przyjętego modelu procesu stosowane są również rozwiązania mające ograniczać zakres przetwarzania i dostęp do danych po stronie Administratora.

18. Zmiany Polityki prywatności

Polityka prywatności może być zmieniana, w szczególności w przypadku:

zmiany przepisów prawa,
zmiany funkcjonalności Serwisu,
zmiany modelu świadczenia usługi,

Website (Service) – the SOSlink website and its functionalities available online, including the online store and forms related to the SOSlink service.
ICE Form – a form made available to the user after purchase or as part of the event model, used to provide data to be used for the preparation of the SOS Sticker and QR code.
SOS Sticker – a personalized product or result of the SOSlink service containing a QR code and data provided by the user within the scope chosen by them.
Organizer – the entity organizing the event within which the user can use the SOSlink service under the event model.
Event – a sporting event or other venture organized by the Organizer, in connection with which the user receives access to the SOSlink service.
Event Model – a method of using the SOSlink service where the user is granted access to the ICE Form in connection with participating in an Event organized by the Organizer.
B2C Model – a method of using the SOSlink service where the user independently orders a product or service directly through the Website.
Data Concerning Health (Health Data) – data concerning health within the meaning of data protection laws, provided voluntarily by the user in the ICE Form.
User – any person using the Website, placing an order, or filling out the ICE Form.

2. General Information

This Privacy Policy sets out the rules for processing personal data in connection with the use of the SOSlink website, the fulfillment of orders placed through the website, and the use of the ICE Form used to prepare the SOS Sticker and QR code. The Policy distinguishes between data processed at the store and payment stage and data provided in the ICE Form, including data concerning health.

3. Scope of the Privacy Policy

The Privacy Policy applies to personal data processed in connection with:

the use of the store and order handling, in particular:
- using the SOSlink website,
- placing orders through the website,
- handling payments, delivery, complaints, and contact with the Data Controller,
- the use of cookies and similar technologies to the extent they are applied on the Website.
the use of the ICE Form, in particular:
- filling out the form used to prepare the SOS Sticker and QR code,
- performance of the service under the event model,
- processing data entered into the ICE Form, including – if provided voluntarily by the user – data concerning health.

IMPORTANT INFORMATION: The SOSlink service generally operates in two stages. In the first stage, the user places an order and makes a payment. In the second stage, after the purchase or within the event model, the user receives access to the ICE Form, where they can voluntarily provide data to be included on the SOS Sticker, including health data. Health data is processed within a separate, narrower scope than the data associated with store operations, payments, or cookies.

4. Service Model

The SOSlink service may operate under two models:

Event model – when the user receives a link to the form in connection with an event organized by the Organizer,
B2C model – when the user independently orders the service or product through the website.

In the event model, the Organizer remains a separate data controller for the data it processes for its own event organization purposes. The SOSlink Data Controller processes personal data independently for the purposes of the SOSlink service.

In the direct model (B2C), the process may take place in two stages: first, the user places an order and makes a payment, and then receives a link to the ICE Form, where they can fill in the data to be placed on the SOS Sticker. Data provided at the order stage and data provided in the ICE Form may be subject to different processing rules, particularly with respect to health data.

According to the adopted model, the Organizer does not gain access to the source data entered into the SOSlink form, but only to the ready result of the service, to the extent necessary for printing, distributing, or issuing the product.

Health data is not part of the payment process or standard store analytics.

5. What Data We Process

Depending on the stage of using the service, we may process the following data:

Data processed at the stage of using the store and order handling:
- identification and contact data, such as first name, last name, e-mail address, phone number, delivery address,
- data related to order fulfillment, payment, delivery, complaints, and settlement,
- technical data related to the use of the Website, such as IP address, device identifiers, session data, or cookies – within the scope resulting from the Website and browser settings.
Data processed at the stage of the ICE Form:
- identification and contact data provided in the ICE Form,
- data necessary to prepare the SOS Sticker and QR code,
- data of the person acting on behalf of a child or another represented person,
- health data provided voluntarily by the user in the ICE Form.

Technical, analytical, and cookie data relate to the use of the Website and the store stage. We do not use them for the description, profiling, or analytics of health data provided in the ICE Form.

6. Sources of Data

As a rule, we receive personal data directly from you.

At the store and order stage, we receive data when you:

use the Website,
place an order,
make a payment,
contact us,
file a complaint.

At the ICE Form stage, we receive data when you:

fill out the ICE Form,
provide data for the SOS Sticker and QR code,
voluntarily provide data concerning health.

Health data is not collected at the stage of the purchase itself, but exclusively at the stage of the ICE Form.

If the service concerns a child or another represented person, the data may be provided by a parent, legal guardian, or another person authorized to act on their behalf.

In the event model, we may furthermore process technical or organizational information related to assigning the form to a specific event or the method of issuing the finished product, if such a model has been implemented in practice.

7. Is Providing Data Mandatory?

Providing data necessary to place an order, make a payment, handle delivery, contact, or file a complaint is voluntary, but necessary to fulfill the order or perform the contract.

Providing data in the ICE Form is voluntary to the extent that the user decides what information they want to place on the SOS Sticker.

Providing data concerning health is completely voluntary. Failure to provide it does not, as a rule, prevent the purchase of the product or the use of the service, but it may result in the SOS Sticker being prepared in a basic variant or with limited functionality.

8. Purposes and Legal Bases of Processing

We process your personal data for the following purposes:

Processing of health data	Processing of health data provided voluntarily by the user exclusively at the ICE Form stage, in order to prepare the SOS Sticker and perform the SOSlink service. The contact form is used for general contact with the Data Controller. Please do not provide health data or other special categories of data therein. Data of this type should be provided exclusively in the designated ICE Form if the user wishes to place it on the SOS Sticker.

Information regarding cookies, technical data, and similar technologies relates to the use of the Website and the store stage. We do not use these technologies to process health data provided in the ICE Form.

9. To Whom We May Disclose Data

Store and order data may be disclosed to entities that support the Data Controller in providing services or fulfilling orders, in particular:
- hosting, IT infrastructure, cloud, or technical support providers,
- providers of tools used to operate the Website and communication,
- payment operators – in the case of paid orders,
- entities handling accounting, legal, or advisory services,
- entities handling logistics, shipping, or distribution of the product,
- entities authorized on the basis of legal provisions.
Data from the ICE Form, including health data, may be disclosed exclusively to entities necessary for the technical execution of the SOSlink service, in particular:
- IT infrastructure and technical maintenance providers of the ICE Form,
- entities supporting the technical preparation of the SOS Sticker or QR code, exclusively to the extent necessary to perform the service,
- entities authorized on the basis of legal provisions.

Health data is not transferred to payment operators or entities performing analytics or marketing activities.

In the event model, the Organizer receives only the finished result of the service to the extent necessary to issue or distribute the product. According to the adopted process, SOSlink does not disclose the source data entered into the form to the Organizer.

10. Do We Transfer Data Outside the EEA?

As a rule, we aim to ensure that data is processed within the European Economic Area (EEA).

If, in connection with the use of specific providers or tools, data is transferred outside the EEA, it will occur exclusively in accordance with legal provisions, applying appropriate mechanisms to legalize such transfer, in particular Standard Contractual Clauses or another appropriate mechanism provided for by the GDPR.

11. How Long We Retain Data

The retention period depends on the purpose of the processing.

a) event model We process data provided in the form for the period necessary to perform the service. According to the adopted process model, this data is, as a rule, deleted no later than 7 days after the event, with the exception of data that must be stored longer on the basis of legal provisions or for the purpose of establishing, exercising, or defending claims.
b) B2C model We store data related to the order for the period necessary to fulfill the order, settle accounts, handle complaints, perform legal obligations, and for the period necessary to defend against claims or pursue claims.
c) health data We store health data no longer than is necessary to perform the service, unless longer storage results from legal provisions or is necessary to protect claims. The withdrawal of consent does not affect the lawfulness of processing carried out before its withdrawal.
d) contact-related data We store data provided in messages or complaints for the period necessary to handle the matter, and subsequently for the period appropriate to defend against claims or pursue claims.
e) cookies We store data related to cookies for a period resulting from the nature of the given cookie, browser settings, Website configuration, or until consent is withdrawn if the given technology is based on consent.

12. Your Rights

You have the right to:

access your data,
rectify your data,
erase your data,
restrict processing,
data portability in cases provided for by law,
object to processing based on the legitimate interest of the Data Controller,
withdraw consent at any time, if the processing takes place on the basis of consent,
lodge a complaint with the President of the Personal Data Protection Office (Prezes Urzędu Ochrony Danych Osobowych).

13. Withdrawal of Consent for Health Data

If the basis for processing health data is your consent, you can withdraw it at any time by contacting the Data Controller.

The withdrawal of consent does not affect the lawfulness of processing carried out before its withdrawal, nor does it affect the processing of other data necessary for order fulfillment, settlements, handling complaints, or performing legal obligations. However, it may result in the inability to further perform the service in the scope that requires the processing of health data.

14. Automated Decision-Making and Profiling

As a rule, your personal data is not used for automated decision-making that produces legal effects concerning you or significantly affects you, nor for profiling within the meaning of the GDPR.

If solutions causing such effects are implemented in the future, the Privacy Policy will be updated accordingly.

15. Data of Children and Represented Persons

If the service concerns a child or another represented person, the data should be provided exclusively by a parent, legal guardian, or another person authorized to act on their behalf.

The Data Controller may process both the data of the person whom the service concerns and the data of the person placing the order, form, or declaration.

16. Cookies and Similar Technologies

The Website may use cookies or similar technologies primarily at the stage of using the website, store, and functions related to order handling.

These files may serve in particular to:

ensure the proper functioning of the Website,
maintain the user’s session,
increase security,
remember user settings,
conduct statistics or analyses, if such tools are actually implemented,
carry out marketing activities, if such tools are actually implemented.

Health data provided in the ICE Form is not used for analytical or marketing purposes and is not subject to profiling using cookies or similar technologies.

17. Data Security

The Data Controller applies appropriate technical and organizational measures to protect personal data against its loss, destruction, unauthorized disclosure, or unauthorized access.

As part of the adopted process model, solutions are also used to limit the scope of processing and access to data on the part of the Data Controller.

18. Changes to the Privacy Policy

The Privacy Policy may be amended, in particular in the event of:

changes in legal provisions,
changes in the functionalities of the Website,
changes in the service delivery model,
changes in providers or tools used by the Data Controller.

The current version of the Privacy Policy is published on the Website.

19. Effective Date

The Privacy Policy is effective as of 02.04.2026.
